Despite extensive research on the numerous risks associated with twin pregnancy, there are still no definitive recommendations regarding delivery of the second twin. Is vaginal delivery reasonably safe for vertex-vertex presentations? Should cesarean delivery be the primary route of choice for all twin pregnancies? And what about intrapartum manipulation of the second twin? This paper reviews the latest findings.
